BEHAVIOR CONTRACT

Students are expected to maintain appropriate behavior and follow directions in order to maintain a fun learning environment

Some activities may require team work , Students are expected to  maintain a cooperative attitude towards team mates and teachers at all times

No fighting, hitting, spitting, name-calling, or any other form of harassing or disrespectful behavior will be tolerated, and the students will be excused from the program for the same.

If the student is repeatedly disruptive , parents will be notified.

If the student behavior is affecting and detracting others from the activities at camp, student will be excused from the program

In case of violent behavior towards other students or teachers , the child will not be allowed to return to the program.

Students are expected to respect technology provided . Parents will be responsible for any damage caused by the student.

EXEMPTION :

​

I understand that STREM HQ is not licensed by the state as a child care center. It is exempted because it is a Summer Camp Program and an extracurricular activity program.

LIABILITY WAIVER:

​

STREM HQ strives to create a safe and fun environment for students, but I understand that accidents can happen. As a parent / guardian of participants in STREM HQ programs , assume all risks both known and unknown even if arising from negligence of others, associated with willing participation in any and all activities through STREM HQ and any and all its affiliated parties.

I understand, and agree to this waiver of liability ,and on behalf of myself and the participants, and on behalf of my heirs, assigns, personal representatives, and next of kin, voluntarily and expressly release, indemnify, forever discharge, and HEREBY HOLD HARMLESS Vigyan Group LLC DBA STREM HQ, its owners, their management, directors, officers, members, employees, agents and ANY and ALL affiliated parties from any claims, damages, cost or expenses including those of attorney fees and medical fees, and liability for any such personal injury, disability, death or loss or damage to person or property resulting from the willing participation in any and all activities through STREM HQ,  and ANY and ALL affiliated parties to the fullest extent of the law.

I understand that I am responsible for making STREM HQ aware of any allergies or medical conditions and limitations that my child may have. i agree to put all medical information in the medical form, and authorize STREM HQ and its faculty to take appropriate measures in case of emergency. I would be responsible for all expenses incurred.

I understand that this is a Release of Liability form. I have read this entire form thoroughly. I am signing this document of my own free will.
